Care Plans

The Manager or a Care Coordinator will visit every Service User to discuss and assess needs.

A plan of care will be developed with needs being the priority.

The plan will include a Health and Safety and Risk Assessment. This will ensure the safety of the carer and our service users.

We will carry out a formal review of care plans every six months.

All agencies concerned with care and welfare may be involved in the review.
